I, a participant of Filipino Ballers Club 2025 One Day League verify that I am in good health and able to fully participate in all events and activities related to Filipino Basketball Club in La Trobe Stadium, Bundoora.

Filipino Basketball Club Pty Ltd and all affiliated entities, are not liable on any claims for injury, illness, or death and/or including less sustained by the participants while playing, practicing, and traveling to and from that may be incurred during all events and activities related to Filipino Ballers Club Pty Ltd.

I, the participant of this competition assume all risks involved.

I, the participant, give permission to Filipino Ballers Club Pty Ltd to use my image, voice, and likeness in photos, videos, or other media for promotional and public relations purposes, including on the internet and social media. I understand these may be shared overseas, and I will not be entitled to compensation or protection under Australian privacy laws if used outside Australia.
