We, participants of Filipino Ballers Club 3x3 2024 One Day Tournament verify that we are in good health and able to fully participate in all events and activities related to Filipino Basketball Club in La Trobe Stadium, Bundoora.

Filipino Basketball Club Pty Ltd and all affiliated entities, are not liable on any claims for injury, illness, or death and/or including less sustained by the participants while playing, practicing, and traveling to and from that may be incurred during all events and activities related to Filipino Ballers Club Pty Ltd.

We, the participants of this competition assume all risks involved.

We, the participants of this competition gives permission for our images to be used in promotional and public relation activities by Filipino Ballers Club Pty Ltd. It includes both print and electronic media, including the Internet and social media platforms. We understand that Filipino Ballers Club Pty Ltd will not be accountable should overseas recipients of the images violate Australian Privacy laws and that I will not be able to seek compensation under the Privacy Act.

Registration fee is $600. 50% required upon submitting this form, the rest of the 50% will paid on the day of the competition.
